The investigation cited numerous failures that led to the crash of the B1 bomber including bad weather, multiple crew failures, wind shear, an organizational culture at the base that tolerated decaying airmanship skills, a lack of discipline, poor communication and inadequate focus on regulations at the base.

Col. Mark Kimball was relieved of his command on Friday. The firing was based on the findings of an Accident Investigation Board.

No replacement for Kimball has been announced yet.
